Share the caregiving duties and be prepared with the CareRelay app. Build a care team around yourself or your loved ones and reduce stress by distributing the workload amongst family, friends, service providers and healthcare providers. We help you get there by sharing tasks, appointments, and tracking medications to communicate, coordinate and collaborate a loved one’s care journey. CareRelay ensures that everyone on the care team is aware of every step in the care plan, including the patient.

An app created by caregivers, for caregivers: We have faced the daunting obligations of caregiving ourselves. Moreover, we consulted with patients, family and healthcare professionals to provide features helpful in a caregivers journey.

Take control of health information to facilitate decisions with family or in emergencies: Liberate health information for action by care team members. Health information is stored in a secure environment in Canada which complies with PHIPA requirements and available to care team members with appropriate permissions. Consolidate information, including diagnoses, medications, appointments, notes, and tests. Family and friends keep information current based on the patients care plan. Share health information with first responders or emergency departments in real-time.

Create and share health information for a loved one:
- Track and assign Tasks to family members, including home, personal, financial or legal
- Track and assign Appointments to family members, including medical, legal or financial
- Track Medications including adherence and prescription renewals
- Enter and share medical information, such as allergies, blood type, diagnosis, conditions, lab results
- Enter important dates, achievements and favourite activities for patient and/or family to remember and action
- Share emergency medical information with healthcare professionals
- Engage third-party service providers to accomplish tasks, e.g. Uber
